If you are a Company / Society and want to vaccinate your employees/ residents at your work place/ residential place, then please send email on following email id: admin@shivamhospitalpune.com mark cc: drrajeshwarn@gmail.com & anjalikalbhor@gmail.com Please mention following details in the email. 1) Name of Company/ Society 2) Address & Contact Details 3) PAN Number/ Registration Certificate of Company/ Society 4) Contact Person Name 5) Contact details: Mobile Number, PAN Card, Aadhaar Card (attachments) 6) Number of employees/ residents (Doses required) 7) Acceptance of charges of 1000 Rs per dose* for Covishield Vaccine (If distance is more or beneficiaries are less than 200, additional charges to cover transport will be applicable) (+ 5% GST if billed in name of company/ society) 8) Readiness to make necessary arrangements at vaccination site. 9) Making decided number of employees/ residents available at decided date and time for vaccination. A MOU will be signed between Shivam Hospital and Company/ Society and follow up will be done with Government authorities to create special COVID Vaccination Site, Workplace COVID Vaccination Center (WCVC) for each Company OR Near to Home COVID Vaccination Center (NHCVC) for Society, on CoWIN Portal in our hospital's Account. Company / Society will cooperate and provide necessary documents in time for getting speedy approvals. Once WCVC / NHCVC is activated on CoWIN Portal, we can plan vaccination on site. We from Shivam Hospital will make vaccines available and provide vaccination service on the site as per vaccines availability and mutually decided dates and time. Payment Terms: 50% Advance, 50% after completion of vaccination within 8days of submitting invoice. We will manage any adverse effects following immunization (AEFI). Minor AEFI manageable at site will not be charged. Major AEFI if any (which is rare) will be managed at actual charges. Together we will win the battle against COVID 19.
