You can register yourself for COVID vaccination by visiting website www.cowin.gov.in and clicking on Register Yourself. Input you Mobile Number. You will get OTP. Enter OTP. You can register 4 members and schedule appointments for them. For taking appointment to Shivam Multispeciality Hospital just type pincode 412308. Select Age group 18+ or 45+, then Select Vaccine Brand: Covishield then select Paid (Free vaccine is available at Government CVCs only) check availability of required session at Shivam Multispeciality Hospital. Select time slot 9am to 11am / 11am to 1pm / 1pm to 3pm / 3pm to 5pm. You will get an appointment confirmation SMS on your phone from NHPSMS. Show this sms at Reception when you come for vaccination. 50% doses will be available for ON SPOT (walk in vaccination) of age group 45 yrs and above. Citizens above 45 yrs can walk in for their first or second dose of COVISHIELD vaccine at present. Reception: Show SMS/ Screenshot/ Print out of your appointment here. You will be given OPD paper with token number. You can have a seat and be comfortable. Your token number will be announced when your turn comes, till then please read the instructions on OPD paper and get your doubts cleared from our doctor. 2. OPD Case Paper: Your data will be registered in Hospital Software and OPD Case Paper will be printed on which 3 days medicines and instructions will be given in printed format (please read them carefully) 3. Registration in CoWIN Portal: If your age is 45 yrs plus and you have not registered on CoWIN portal then our registration officer will get your data registered on CoWIN portal. If you have already registered and also scheduled appointment on CoWIN portal then you can skip this step. 4. Verification: Our verification officer will verify your identity proof, take your and your ID card Photo and upload it on the CoWIN Portal. 5. Payment: You will pay Vaccination Charges as per Vaccine Brand at payment counter by Cash or any mobile payment app. Get your receipt and OPD Case Paper. 6. Vaccination: Vaccination will be done in batches of 10 beneficiaries. We will open new vial only when 10 beneficiaries are ready for vaccination to ensure cold chain maintenance and minimise wastage. Our trained staff will vaccinate you and give instructions about post vaccination care and medicines and ask you to wait for observation for 30 minutes. 7. Observation: You will wait in Waiting Hall for 30 minutes. If you notice any discomfort or adverse effects our observation officer will take you to our Casualty Department for further management. 8. Feedback: While you are waiting for observation our Public Relation Officer will take your feedback.
