HRCT CHEST PLAIN ONLINE APPOINTMENT BOOKING PROCEDURE Shivam Multispeciality Hospital, Fursungi, Pune 412308 (Please use Android Mobile or PC (Do not use iPhone) • Open Web Browser (Google Chrome/ Firefox etc.) • Type website: www.shivamhospitalpune.in • Click on BOOK APPOINTMENT button • Appointment Window will open (if appointment window opens partially & if only heading tabs are seen: then click on Have an Enquiry tab & then click on Book Appointment Tab) • Select Purpose: HRCT Chest Plain • Select Doctor: Shivam Radiologist, Shivam Hospital • Select Date: by clicking on Right or Left Arrow • Time Slots: Available time slots will be displayed in dark, click on required time slot • BOOKING DETAILS (Time Slot): Fill in your Name, Mobile Number, Email ID, Gender (Male/Female), Age • HRCT Chest Plain Scan Appointment Fee: 2500 Select Pay at Clinic (Avoid paying online) • Then hide or pull down keyboard, behind that you will see CONFIRM BOOKING button • Click on CONFIRM BOOKING and wait for few seconds • PAY AT CLINIC Window will open: Enter your Short Address & Pin code (Pull down or hide keyboard) • Click on CONFIRM Button once only it will become red (Do not click again and again) wait for few seconds • Appointment Confirmed Message will open, Check details (wait, do not click on done immediately) • TAKE A SCREENSHOT or Photo of this Appointment Confirmation message (So that in case you don’t get SMS, you can show screen shot or photo) • Now Click on DONE (you will get a SMS and Email of Appointment Details) • Sometimes due to technical issue you may not get SMS but you will definitely get an Email Confirmation in few minutes Please visit Shivam Hospital on booked Date and time and show SMS or Screenshot or Email at Reception of Shivam Mutispeciality Hospital, Taravadi, Fursungi, Pune 412308.
